Complete this form to apply to have your health care site designated as a Yellow Fever Vaccination Centre (YFVC). Each applicant site must complete an Attestation signed by the Nominated Health Care Practitioner (NHCP) confirming that the health care site meets the requirements to be designated as a YFVC.
This form is to be used for both new applications and renewals of current sites.
